Community Care Coordination is: Providers working together to provide better care for you.

Community Health Workers/Resource Navigators connect people to resources in their community that help improve health. They work to connect individuals to:

· Medical, mental health, dental, or vision care

· Resources to help manage finances

· Resources for food, transportation, housing needs
