Every three years, Community Health Partners (CHP) and our Sioux County healthcare partners work with community members to identify the most important factors affecting health in our county. This collaborative process, called the Community Health Assessment (CHA), helps us understand local strengths, needs, and gaps that influence the health of all who live, work, learn, and play in Sioux County. Healthcare partners include Hawarden Regional Healthcare, Hegg Health Center, Orange City Area Health System, Promise Community Health Center, and Sioux Center Health.

Once the CHA is completed, CHP and each healthcare partner develop a Community Health Improvement Plan (CHIP). These plans outline targeted strategies to address identified needs and build on existing community assets. Together, the CHA and CHIP represent vital local action to promote and protect the health of Sioux County residents.

This work also aligns with long-standing statewide efforts to improve population health. For more than 30 years, Iowa communities have used the CHA–CHIP process to guide planning and action. In addition, federal requirements for nonprofit hospitals to conduct community health needs assessments every three years create a valuable opportunity for hospitals and public health to join forces. When data, resources, and expertise are shared, collaboration deepens, new partnerships emerge, and—most importantly—the community benefits from a coordinated approach to achieving a healthier future.
