Communicable Disease Surveillance

Community Health Partners, with the assistance of local medical providers and the Iowa Department of Public Health (IDPH), provides communicable disease surveillance in the community. This means watching the community for patterns of illness and following up when someone in the community becomes ill with a confirmed reportable communicable disease or infectious condition.

Confirmed means that the individual has been to the hospital or medical clinic and lab results have proven the person has the disease or infection.

Reportable means that the disease or infectious condition is required, by the Center for Acute Disease and Epidemiology (CADE), to be reported to the state health department. Reportable diseases often require further follow up by the local public health department to try to find the source of the disease or infection to prevent an outbreak, or further spread of the illness or infection.
