Many things besides health care and illness affect our health. A Community Care Coordinator can connect you to resources in the community that can help make you healthier. Anyone who has needs that affect their health — like housing, transportation, behavioral or mental health care, access to healthy foods or social connection — can get help from a Community Care Coordinator. Your medical provider can refer you to their Community Care Coordinator when you are seen in the medical clinic or hospital, or you can call Community Health Partners (712-737-2971) and talk to one of our Community Care Coordinators.
